Yes. Hidalgo County has a large number of Medicare Advantage plans available and many of them carry no monthly plan premium. This area is one of the more competitive Medicare markets in the country.
The more useful question is what zero premium means, because it does not mean free coverage.
What zero premium actually means
It means the plan charges no additional monthly premium beyond what you already pay. You still pay:
- Your Part B premium, every month, without exception
- Copays and coinsurance when you use care
- Drug costs according to the plan's formulary
- Any IRMAA surcharge if your income is above the threshold
The plan is funded by the fixed monthly payment Medicare makes to it on your behalf. That is why it can operate without charging you a premium, and it is entirely legitimate.
How the plan makes the math work
A zero premium plan manages cost in ways you should know about before enrolling.
- A tighter network, often HMO rather than PPO
- Prior authorization on more services
- Referrals required to see specialists
- Copays concentrated on the services people use most
- A higher annual out of pocket maximum than a plan that charges a premium
None of that is hidden. It is simply the trade being made.
The number to look at instead
Compare annual out of pocket maximums, not premiums. That figure is your worst case year and it is the honest measure of how much protection you are buying.
A zero premium plan with a high maximum and a plan with a modest premium and a low maximum can differ by thousands in a year with a hospitalization. In a healthy year the zero premium plan wins easily. Insurance is priced for the bad year.
Then check the two things that decide it
- Are your doctors and your hospital in that specific plan's network for the coming year? Ask the billing office by plan name, not by carrier.
- What do your actual prescriptions cost on that plan's formulary at your actual pharmacy? Two zero premium plans can price the same medication very differently.
Those two checks eliminate most bad matches before you get to anything else.

If the plan is free, how does the company make money?
Medicare pays the plan a fixed monthly amount for each enrollee. The plan manages care within that budget. Nothing improper about it, but it explains the networks and the prior authorization.
Is a zero premium plan worse than one with a premium?
Not automatically. It usually means a tighter network or a higher out of pocket maximum. For someone healthy with in network doctors it can be the better value.
Do I still pay Part B on a zero premium plan?
Yes, always. That never goes away, unless a Medicare Savings Program pays it for you.
